We really appreciate our relationships with all of our current teachers, so please be advised that we will not remove any teacher from our In-Store Teacher Referral List without express instruction from that teacher or in the case that we find we have outdated contact information. We will begin checking contact information Quarterly beginning January 2012, so please make sure to check in with us from time to time if there are any changes to your contact, scheduling, availability, fee or affiliation information. If for any reason our association to you as a teacher proves to be unsatisfactory to you, please let us know so that we can correct any problems or remove you from our Referral List if necessary.

• Current contact information, including Telephone Number and Email Address.
• Type of Instrument(s) Taught, Range of Student Level, Age Preferred & Accepted
• Primary Location of Lessons & Willing to Travel?
• Lesson Fee Rate Structure + Extra Cost for Traveling Expense specified
• Résumé (to include): Applicable Teaching Experience, Colleges & Degree Majors & Minors, Training & Certification, and Community Involvement
